Senior Preferred contracts with OutcomesMTM™, a Cardinal Health company, to offer Medication Therapy Management (MTM)services to Senior Preferred members. Through OutcomesMTM, specially trained Personal Pharmacists are identified in Senior Preferred’s service area.

These pharmacists do much more than dispense medications. They are there to help you get the best results from your medication while keeping out-of-pocket costs down. By using an OutcomesMTM Personal Pharmacist in your community, you can get the extra attention you need to keep your medications on the right track.

As a Senior Preferred member, you are eligible for these important services at no additional cost if you meet all of the following criteria –

  1. You must have a total drug cost of more than $4,044 a year on medications
  2. You must have 2 or more chronic diseases
  3. You must take 6 or more medications for chronic illness

Among the MTM services available –

  • Comprehensive Medication Review (CMR):
    Your OutcomesMTM Pharmacist will meet with you face-to-face or via phone to review all your medications. This will help identify any duplications or conflicts and help to organize your medication schedule. Following the CMR, you will receive a Medication Action Plan and Personal Medication List.
    • Estimated Time Commitment: about 30 minutes

  • Prescriber Consult:
    Your OutcomesMTM Pharmacist will work with you and your doctor(s) to resolve any problems found with your medications.
    • Estimated Time Commitment: 5 to 15 minutes

  • Medication Adherence Guidance:
    Your OutcomesMTM Pharmacist will ask about how you take certain medications to ensure the medication is working well for you and discuss ways to make it easier to take your medication.
    • Estimated Time Commitment: 5 to 15 minutes

Member Notification

  • If you qualify for MTM services, you will be contacted through the mail. Your Personal Pharmacist may also contact you face-to-face or via phone.
  • At least four times a year, we identify members who meet eligibility criteria for MTM services by reviewing their medication history.
  • Participation in our MTM program is voluntary for eligible members. If you are eligible for MTM services but would like to opt out of the MTM program, please contact our Customer Service Department.

Are all pharmacists in the Senior Preferred network also OutcomesMTM Pharmacists?

  • Not all pharmacists in the Senior Preferred network are OutcomesMTM Pharmacists. OutcomesMTM Pharmacists have finished a special training course to provide these extra services. This course is open to any pharmacist. Ask your regular pharmacist if they participate.

Can I use my regular pharmacy and still visit an OutcomesMTM Pharmacist too?

  • Yes. Your OutcomesMTM Pharmacist is an added value of membership. You may continue to use any Senior Preferred participating pharmacy for your prescriptions.

Will I pay additional costs for using an OutcomesMTM Pharmacist?

  • No. These services are offered at no additional cost.

Do I need an OutcomesMTM Pharmacist?

  • While OutcomesMTM Pharmacists can benefit all members, those with diabetes, high blood pressure, or other conditions treated with medications may see greater benefit.

Can an OutcomesMTM Pharmacist save me money?

  • Yes. Similar medications can have large cost differences. Your OutcomesMTM Pharmacist can help you take the least costly, most highly effective medications.

Will my doctor know if any changes need to be made to my prescriptions?

  • Yes. Your OutcomesMTM Pharmacist may make recommendations to you and your doctor(s) – but only your doctor can change your prescription.

Will I be required to use an OutcomesMTM Pharmacist?

  • No. Your OutcomesMTM Pharmacist is an added service of membership. They are there to help, but you are not required to use them.

How do I get started?

  • Ask your pharmacy if they participate with OutcomesMTM or call Customer Service to find an OutcomesMTM Pharmacist in your area to schedule your Comprehensive Medication Review (CMR).
