Senior Preferred has disease management programs available to offer support and education to members who have ongoing health conditions. A registered nurse works with each program. We send information in the mail and are available to members by phone.

Senior Preferred offers a Complex Case Management (CCM) service. This service is for members who have high risk medical conditions, many hospital visits or complex medical needs. A registered nurse works with eligible members by phone to coordinate healthcare services and support members in learning about health conditions. There is no fee to participate.
