Your Plan Documents
To review benefit documents specific to your Senior Preferred plan, visit the My Plan page and enter your member identification (ID) number, located on your member ID card.
By creating an account to access our Member Login, you will be able to view your plan documents and medical claim summaries. You can also request a new member identification (ID) card and submit questions to Customer Service online.
We offer information and programs to support member health. Visit our Health Information and Programs page to find healthy recipes, search for health information and learn more about our Complex Case Management and Disease Management programs.
Three times each year, we mail a newsletter to all members. It provides information to help you understand your Senior Preferred benefits. We also include health-related articles to help members learn about staying well. You can view the most current and past newsletters here.
If you misplaced a form and need another copy, you can call Customer Service or locate a copy here to print.

You, your prescriber, treating provider, or authorized representative may request an initial decision or appeal using the contact information on this page.
