A customer service representative is available to assist Quartz Medicare Advantage (HMO) members Monday through Friday from 8 a.m. to 8 p.m. In addition, from October 1 through March 31 we are available Saturday and Sunday from 8 a.m. to 8 p.m.

We can schedule a time for you to meet with a representative to review our plan options. For people who are deaf, hard of hearing or speech impaired, please call TTY 711 or toll-free (800) 877-8973, or you may call through a video relay service company of your choice.

To meet with a customer service representative at our Quartz Onalaska location at 2651 Midwest Drive in Onalaska, WI, please call (800) 394-5566, TTY / TDD 711 or (800) 877-8973 to schedule an appointment.

We are available Monday through Friday from 8 a.m. to 4:30 p.m.

You may also stop in at the Resource Center located in the main lobby of Gundersen Clinic at 1836 South Ave in La Crosse, WI, where no appointment is necessary.
